We thank God for the sacrifice and service of so many who willingly give their lives for our protection, safety, and freedom. Student Athletic Forms Needed for Winter Sports Students who were not involved in a fall sport but are planning to try out for a winter sport must have the necessary athletic forms on file (WIAA green Physical Card or WIAA orange Alternate Year Card, theKMLHS blue Compliance Form, the yellow Parent/Athlete Concussion/Sudden Cardiac Arrest Agreement Form, and the white Trainer Authorization Form). Athletes will not be allowed to try out if the forms are incomplete. Please contact Mrs. Amy Nass (262.677.4051 x4008; amy.nass@kmlhs.org) for verification or with questions. Athletes who participated in a FALL SPORT would already have the necessary paperwork submitted. Forms can be obtained from the Activities Office or from kmlchargers.com/athletic-paperwork.
